Case 0035: Dual-Molar Occlusal Restoration under Absolute Isolation (Teeth 36, 37)
  • Primary Clinical Pathology: Deep Class I occlusal carious lesions (caries media) undermining the functional cusp slopes and fossa networks of the lower left first and second molars.
  • Isolation and Soft-Tissue Retraction Matrix: Mandibular left quadrant isolation anchored on tooth 37 with a heavy-gauge molar clamp. The rubber dam sheet was fully inverted into the sulcus to prevent moisture contamination from the patient’s breath.
  • Biomimetic Material Architecture: Prebond total-etch adhesive network, President Dental Flow lining layer, and Dynamic Plus micro-hybrid universal composite for high wear resistance.
  • Clinical Execution Strategy: Following carious tissue removal, the cavity configurations exhibited a high C-factor. A thin layer of President Dental Flow was placed along the pulpal floor to cushion initial polymerization contraction. Dynamic Plus was then placed using an oblique layering technique, contouring and curing each cusp slope independently to replicate native occlusal anatomy and minimize internal shrinkage stress.
